- PURPOSE
-
- Home Health Licensed Practical Nurse provides nursing services to individuals and families in their residence under the supervision of a registered nurse.
-
- KEY RESPONSIBILITIES
-
- Performs prescribed individualized patient care to patients as delegated.
- Participates in patient case conferences as appropriate.
- Assists the RN in the preparation, evaluation and modification of the patient's plan of care as appropriate.
- Observes, documents and reports to the RN as appropriate, the patient's physical and emotional needs and reactions to care.
- Provides nursing care in a way that considers patient's privacy, comfort and safety.
- Demonstrates the ability to instruct patients in the home according to the plan of care.
- Performs procedures based on the Agency's policies and procedures, including but not limited to all educational requirements.
- Accepts telephone orders from physicians in accordance with scope of practice.
- Documents patient information in a clear, legible, timely and accurate manner according to policy. Maintains all licenses/certifications as required.
- Participates in the Agency's continuous quality improvement activities.
- May participate in selected research projects as appropriate.
- Utilizes infection control practices with the delivery of patient care services.
- Works collaboratively with peers and other healthcare workers.

- MINIMUM QUALIFICATIONS

- A graduate from an approved school of practical nursing.
- Must be licensed in VT & NHÂ

- REQUIRED LICENSE/CERTIFICATIONS/REGISTRATIONS SKILLS (Tracked at the department level)
